    Proudly part of the Better Connect partnership, provide our Clinical Care Coordination services  in the Sunshine Coast, Wide Bay and Central Queensland regions, assuring that people in these areas are afforded the absolute best support for their mental health needs. 

    If you’ve experienced or are experiencing severe mental health conditions, our Clinical Care Coordination services can help. When you are referred to Open Minds Clinical Care Coordination, you can expect a structured and supportive journey tailored to your unique needs. 

    • What does a typical pathway involve?
      1. Initial assessment: this will help us understand your mental health condition and any related physical health needs
      2. Develop a personalised care plan: this plan will include evidence-based psychological interventions, such as Cognitive Behavioural Therapy and Acceptance and Commitment Therapy
      3. Coordinated care: we will coordinate with various healthcare providers and complementary services to ensure you receive comprehensive care.
      4. Ongoing support and review: receive continuous support through regular follow-ups and reviews of your care plan.
      5. Transition to tertiary care: if necessary, we will facilitate a seamless transition to tertiary care services
      6. Quality improvements: we will continually enhance the service we provide to ensure the best possible outcomes for you
    • What is Better Connect?

      Better Connect is a partnership of nine established, not-for-profit service providers. By working together, we are transforming the way mental health, alcohol and other drugs (AOD) and suicide prevention services are delivered. Better Connect offers free mental health, AOD and suicide prevention services so children, young people and adults can receive the right care, in the right place, at the right time.

      For a referral, please contact the Medicare Mental Health Centre Intake, Assessment and Referral phone service on 1800 595 212. The more information is always better, so that they can properly understand your needs and provide the right support for you.

      They will discuss your needs to see how best to help you. This process will not take long, and they will connect you with the appropriate support services.
