What is Support Coordination?

Support Coordination is an NDIS funded Support, designed to assist you to make sure you get the most out of your NDIS funding package. Support Coordinators work together with you to explore what options are open to you

Support Coordinators assist you to build skills to understand your plan, and most importantly help you put the plan into action.

If you have Support Coordination in your approved NDIS plan, they will work with you to understand all the confusing parts about your plan, and make sure you get the right supports to help you to:

  • Maintain relationships
  • Manage service delivery tasks
  • Live more independently
  • Be included in your community

How does a Support Coordinator help?

Your Support Coordinator will help by working closely with you, and your family if you want, to recommend support services from providers that are best suited to your individual needs. They will:

  • Help you to get good value from your plan and support you with plan reviews
  • Help you to setup the NDIS portal
  • Support you to contact recommended providers
  • Support you to confirm bookings
  • Explain Service Agreements
  • Help you to learn how to coordinate your needs on your own if that’s what you want

We understand how challenging it can be to navigate the NDIS and the range of providers and services out there. Our trusted Support Coordinators can help you navigate this.

Open Minds Support Coordinators have a real team-based service, so if your Support Coordinator is away and you need urgent assistance, another Support Coordinator will help you.
